Liability & Participation Acknowledgment *
I acknowledge that participation in any sport or physical activity carries an inherent risk of injury. I understand that Meridian Community Unit School District 223 and the Stillman Valley Volleyball Program are not liable for injuries that may occur during camp activities. I understand my child is voluntarily participating in this camp.

Medical Authorization *
I grant permission for camp staff to obtain appropriate medical treatment for my child in the event of illness or injury and to administer basic first aid as needed. In the event of an emergency, every effort will be made to contact a parent or guardian. If contact cannot be made, I authorize camp personnel to secure proper medical care for my child.
